ALTERNATIVE FORMAT OF TEXTBOOK FORM
In order for the Community College of Aurora (CCA) to provide this service, it is required that we be in compliance with the Colorado/Wyoming Consortium Alternative Text Exchange Agreement. You may be a candidate to receive this service provided that you have:
1. Completed the Needs Assessment through the Accessibility Services Office (ASO).
2. Provided the appropriate documentation of your disability and an alternative textbook format is determined to be an appropriate accommodation for your situation.
3. Submitted the original receipt as documentation that you own a physical copy of the textbook you are requesting in alternative format (you are required to provide the original receipt to be held on file until you return the alternate format at the end of the semester).
4. Submitted the completed textbook information requested on this form
Your request may take time to process as these are the following steps we go through in order to provide you with the alternative format of a textbook:
- CCA library (however, the department may have changed editions)
- Recordings for the Blind and Dyslexic (RFB & D)
- Other educational institutions
- Publisher
- As a last resort, we may ask you to cut the binding from your textbook so that we can scan it into electronic format for you. If you need this immediately, we can give you a copy that is not edited.
The type of alternate format of the textbook will determine which type of equipment you will need to access this information. Equipment is available to use while you are in the Computer Access Center. If you are not familiar with this equipment, you will want to make an appointment to receive free training. Some equipment can be checked out at no charge. If your alternate format requires the use of a screen reader software, information is available for purchasing this for your home computer.
Alternate Format Preference (Please indicate by checking your first preference. Every effort will be made to accommodate that preference. Training on using the various formats is available at no charge upon request. Software and hardware is available for use in the Computer Access Center):
