Directions

• Read the Veterans Information Sheet.

If you have not previously applied for and received VA benefits:

1. Print, complete and sign the Veterans Intake Sheet.
    
2. Download and complete VA Form 22-5490 (Application for Survivor's & Dependent's Educational Assistance).
    
3. Bring the completed forms along with a copy of your marriage or birth certificate as appropriate to the VA advisor.
    
4. If you have taken college courses at another school, you will need to request official copies of your transcript(s).

If you have previously applied for and received VA benefits:

1. Print, complete and sign the Veterans Intake Sheet.
    
2. Download and complete VA Form 22-5495 (Request for Change of Program or Place or Training Survivor's & Dependent's Educational Assistance).
    
3. Turn in completed forms to the VA advisor.
    
4. Request that official transcripts from all colleges/universities you have attended be sent to CCA.

Once these forms and documentation have been completed and turned in and you are enrolled in classes, that information will be forwarded to VA with an Enrollment Certification and VA will process your claim.
