Temporary Services

Note: If your injury is more than 6 months in duration, do not submit this online form. Instead, request services through the online Needs Assessment form for accommodations.

A temporary injury is defined as an injury of less than 6 months in duration and does not qualify for accommodations under Section 504 of the Rehabilitation Act of 1973. However, on a case-by-case basis, temporary support services may be approved with the intent of lessening the impact of the injury on the student in a learning environment.

Determination will be based upon:

1) The student's request through the Temporary Services online form
2) Documentation of functional impact of injury supporting the temporary services request
3) Brief temporary services interview (can be by telephone, e-mail, or in person)

Because services may require time to process, it is recommended that students complete this online form and submit documentation of functional impact as soon as possible. Please contact us for questions, comments, or assistance with our online forms at or 303.361.7395.
