CentreTech Make-up Test Submission Form
Please complete this form to have a test proctored at the Library at CentreTech.
Note: The Make-up and Accommodation Testing Service is to help students who need accommodations or who occasionally are unable to meet a quiz/test date. If more than 20% of students in a class require make-up exams, the instructor is asked to host their own. Due to the limited space and personnel, this service is not available for entire classes.
